There are currently 4 forms ( a- e ) required for participation in the 2008 Camp del Corazon sessions. If you are interested in joining us, please submit the completed application forms by mail.

The Parent or Legal Guardian must complete and sign the:

	General Information Form (a)
	Release Forms (b)
	Camper Expectation Form (c)
	Camper Profile Form and demographics (d)

Your Pediatric Cardiologist must complete, sign, and return the:

Cardiology Medical Form (e)
