Camp del Corazon is a registered 501(c)3 nonprofit organization that provides year-round experiential opportunities for children faced with the challenges of growing up and living with heart disease. We provide a residential summer camp experience for children ages 7-17 as well as programs that offer opportunities for growth, education and support for children, young adults and the families who are living with heart disease.

All personal information will be saved from year to year – if there have been changes to your medical and/or personal information you will need to update them on a yearly basis.

Upon submitting your application, your Pediatric Cardiologist will receive an email from us alerting them that you have applied to camp and to complete the Camper Cardiology Form as soon as possible. Please be sure to have an up-to-date email for your Pediatric Cardiologist. If your Pediatric Cardiologist does not have an email address, please download and print the Cardiology Medical Form and submit it to your Pediatric Cardiologist.

Your Pediatric Cardiologist must complete, sign, and return the: Cardiology Medical Form

If you cannot complete the online application above, please download hard copy application below. There are currently 6 forms ( a- f ) required for participation in the Camp del Corazon sessions. If you are interested in joining us, please submit the completed application forms by mail.
